This adjustable bed frame is an all-steel support system that provides a sturdy, comfortable foundation for sleeping. You'll enjoy excellent support for your back, legs, hips and spine. The steel bed frame features versatile, easy-to-use brackets that connect to bolt-on headboards and footboards, allowing for easy set up. It can also be used to convert a baby crib into a full-sized bed. The Twin and Full bed frames include four legs with adjustable steel gleam glides, as well as the King and Queen bed frames, which include six legs and a center bar for extra support. The adjustable bed frame features a no-tools design and key-slot cross arms for hassle-free assembly.

does the width adjustment on this frame allow attaching a 48 inch wide matress to a 54 inch wide headboard

I adapted an 1870's narrow french bed with this frame to accept a new queen mattress and box spring. I set the frame for the mattress size then used 1X4" fir strips across the headboard and footboard flanges, fastening them with bolts.
Drill holes in through the strips and fasten through the BACK of the strip with wood screws to any appropriate location in the FB&HB. The original wood side rails were not reused.

No. The frame will adjust to only set widths (38, 47, 53). When the width is adjusted to 47 inches the brackets will not reach the 54 inches you need.

For larger beds, there needs to be more support. Walmart offers another frame that in fact can be used for queen size beds.

Mine is driectly on the frame - no problems. It's certainly firm enough that you don't need a box...

It depends on how firm your mattress is. I bought a faux temperpedic and it is too cushy to hold firm and sagged and I had to get a board to put under it and figure up a brace for the middle with cement blocks. It works but I would advise a box.

if i only have a headboard, can the footboard frame be removed?

under features and specifications of this product it says if you only have a headboard look at "complete the look", however there is no such thing on this page. I recently bout a headboard from walmart and would only like to put the headboard on the bed frame with no footboard

Yes, I purchased this and I have a king with split box springs. As long as you have the dimensions measured correctly it should fit.

Yes. The legs (it doesn't have wheels) are not installed in the frame when it is delivered. If you choose to leave them off it is up to you, but I put them on as extra support for the bed as the frame is not going to be strong enough to support the weight on its own.
The legs are adjustable to just about any height that you would need up to about 8 - 9".
