Refurbished means it was previously owned by someone else and returned. Since the return period for phones is typically 30 days, it's safe to say the person owned it for no more than 30 days. Sometimes they buy phones, open it up, and return them the same day.
They can either not like it for any reason, design, speed, price, service; or yes, something can be wrong with it. Regardless of the reason, the phones are sent to an authorized refurbishing center where every part of the phone is inspected (hardware, software, internal function, and cosmetic look for scratched and prints).
